As of February 2007 a DHS application form must be submitted with all new projects regardless of origin. For example all university studies must be submitted with the common application form not the university application form.
Amendment to the Guardianship and Administration Act 1986. Please refer to information under appropriate heading.
As of February 2007 the Participant Information & Consent Form/s must be presented in the VMIA format. No other format will be accepted.
Eastern Health local guidelines for ionized radiation are now complete.
Research involving the services of an Eastern Health department/s must include a letter of support from that department/s with the original application and amendments (if applicable). For example, a letter of support must be submitted from Pathology if the project involves the services of that department. Please provide a letter from the department head (or designee) OR print off Question 1.40 from Module One.

Mental Health projects must be submitted with a DHS Common Application Form and letter of approval from the Mental Health Academic & Research Committee. The researcher is responsible for submitting the full application to the Eastern Health Research & Ethics Committee.
