Application Forms and Guidelines

The aims of this research project application package are to:

• facilitate the efficient and thorough review of research involving humans;
• ensure that the research is conducted in accordance with the NHMRC National Statement on Ethical Conduct in Human Research (2007).

How To Use This Application Form

The form consists of a number of modules, all of which have been revised to conform to the National Statement on Ethical Conduct in Research (2007). All applicants must complete:

Applicants undertaking specific types of research must also complete the additional module/s relevant to their research.
Each module includes guidelines for answering the questions. We recommend that applicants read the guidelines before completing the form.
To submit an application, download the appropriate modules (using Word) for completion.

The Committee requires all Participant Information and Consent Forms to follow the format and wording as recommended in the common application form guidelines and templates. Non-adherence to these standards will result in rejection of the document and a delay in study approval.
