Eastern Hospital Admission Risk Program (HARP) is a service for people who may benefit from receiving services in a community setting rather than a hospital setting. The target population for Eastern HARP is those people with defined chronic diseases & complex needs who are most likely to benefit from integrated care and have the potential to reduce avoidable hospital use. This includes:
- People with chronic heart disease
- People with chronic respiratory disease
- People with diabetes
- Older people with complex needs
- People with complex psychosocial needs
Evidence shows that these groups are ‘people who frequently use hospitals or are at risk of hospitalisation’. The focus is therefore on ensuring healthcare needs are met to reduce/eliminate avoidable hospital admissions. HARP integrates the essential partnerships between the GP, community, hospital staff and the client and their carer. Timely intervention and care planning involving the client, carer, clinicians, care coordinators, the patient’s GP and other medical specialists are key components of the program. To create an integrated, effective and sustainable chronic disease and complex needs program, which provides the right care in the right setting, to reduce avoidable hospital admissions and contribute to better health outcomes for the Eastern Population
The 4 key objectives of HARP CDM for the defined target groups are:
- To improve outcomes for the chronic disease population;
- To provide integrated seamless care within and across hospital and community sectors;
- To reduce avoidable hospital admissions and Emergency Department presentations; and
- To strive for equitable access to healthcare.

 The Eastern HARP streams provide varied services to ensure that all client needs are met. HARP clients can move freely between streams should the need arise. The HARP streams provide the following services:
- Psychosocial – Care Coordination, Psychology and Psychosocial Clinical services
- Aged – Care Coordination, access to Geriatricians, Multidisciplinary health services and Community Case Management
- Medical/Chronic disease – Care Coordination and specialist clinical nurse support, Multidisciplinary clinics and General Practice based services
- Allied Health – Physiotherapy, Occupational Therapy, Speech Pathology, Dietitian and Pharmacy
 Eastern HARP is a geographically dispersed program which encompasses a number of Local Government Areas (LGA) including Yarra Ranges, Manningham, Boorondara, Knox, Maroondah, Whitehorse and a small part of Monash.  Referring to HARP
Referrers to Eastern HARP can be hospital inpatient services, hospital outpatient and ambulatory services, community service providers, General Practitioners, potential clients and their carers.
Referrers can contact Eastern HARP Access by phoning 1300 661 141 or via e- referral (www.s2s.org.au) or fax 1300661145 using the Service Coordination Template Tool (SCTT). General Practitioners are able to use the HARP GP referral form. Referrals via Eastern e-referral, email, fax and phone are all accepted by HARP Access and should contain the following SCTT information:
- Name & Date of Birth
- Contact details (e.g. address, telephone number/s)
- Gender
- Interpreter services required
- GP address and contact details
- Medical condition/chronic illness
- Date of and reason for referral
- Referrers contact details (including fax number)
