Description

The Eastern HARP – COPD multidisciplinary clinic will optimise health outcomes for people living with COPD. The clinic provides a comprehensive medical assessment in addition to physiotherapy and nutrition review. Patients will the referred for Home Medication Review if needed.
The COPD Clinical Nurse Consultant co-ordinates a case conference with the team at the close of the clinic, a COPD action plan including patient goals to optimise health outcomes and develop self-management strategies will be developed with the team. |

-Lung function test (Spirometry and Gas transfer)
-Echocardiogram if indicated
-CT of the chest |
Inappropriate referrals for
this specialty |
Anyone without a confirmed diagnosis of COPD.
To be eligible for HARP services patient must be at risk of admission to hospital or have had 2 presentations to the Emergency Department in the past 6 months. |
