Description

Independent, comprehensive multi-disciplinary assessment of the physical, medical, psychological and social needs of clients and their carers
Evaluates the needs of clients for restorative care and/or rehabilitation
Assists clients in accessing the services they need
Provides information about aged care services and ageing processes
Determines eligibility for designated Commonwealth Government funded aged care programs facilities
Assess in clients homes, public and private hospital and residential aged care facilities
