Nurses• Birralee Maternity Service

Birralee is a Level 2 Maternity Service at Box Hill Hospital with over 2000 women birthing per year, and is purpose built specifically to facilitate a team based approach to maternity care. All models of care are team-based and woman centred to enhance continuity of care; and depend on a multidisciplinary and collaborative approach to care management. The model of care for each woman will be determined by her choice and her clinical risk, such as Midwife shared care 59%, GP shared care 22%, Know Your Midwife caseload 9%, and Consultant care 8%. Specialist clinics are available to women requiring a case management approach such as drug and alcohol, or mental health issues.  (see also: Birralee Maternity Service webpage)

Know Your Midwife Model of Care

Birralee midwives work across the continuum of midwifery practice. There is a strong commitment to assist midwives who may be unfamiliar with this approach to develop their skills. We support this through orientation, mentoring and continuing education.

Birralee provides clinical placement to midwifery students undertaking postgraduate and direct-entry programs, and in addition has a Graduate Midwifery Program for consolidation of practice (link to GMP site) Students and Graduates are allocated within a team environment, which has proved successful in skill and knowledge development while maintaining a women centred approach to midwifery practice.

• Neonatal Nurses / Midwives

The Special Care Nursery is a 12 cot high Level 2 nursery, with a combination of core staff and team midwives on rotation. Core nursery staff are required to have a Level 2 or 3 Neonatal qualification. This is a very busy unit as the proximity to the Level 3 facilities results in transfers to Birralee for ongoing care. There is an active staff education program including orientation, mentoring and ongoing education such as NETS study days.

• Coronary Care Unit

Box Hill Hospital’s dynamic Coronary Care Unit provides best practice management of acute cardiac conditions including the management of intra-aortic balloon pumps, invasive monitoring, non-invasive ventilation and provides post procedural care for coronary angiography, angioplasty & stenting, ASD and PFO closures, inserting of temporary/permanent pacemakers, automatic implantable cardiac defibrillators (AICDs), resynchronization therapy, and electrophysiology studies.

We offer a supportive work environment, are team orientated and patient focused, with ongoing professional development opportunities to committed cardiac nurses. We have a dedicated Cardiac Educator and also provide a resource nurse on the floor for clinical support. We are actively involved in clinical based cardiac research. We provide an interactive phase two Cardiac Rehabilitation program.

Our Coronary Care Unit is an integral part of the Eastern Health Cardiology services.

• Intensive Care Unit

Box Hill Hospital is a level 3 tertiary referral centre. The Intensive Care Unit is a 12 bed facility, 9 of which are currently open.

ICU services include invasive and non-invasive mechanical ventilation, invasive cardiovascular monitoring, vasoactive drug therapy, extracorporeal renal replacement therapy and intra-aortic balloon pump therapy.

The unit has an open plan layout and is well equipped and resourced.

The philosophy of the Unit is that ‘we believe that nursing is the art and science of caring. We care for our patients and we extend that care to patient’s relatives and one another.’ The unit has a friendly and supportive atmosphere. We demonstrate that support in the following ways.

Clinical Support

There is a supernumerary resource or support nurse on all day and evening shifts that offer clinical assistance and mentoring to novice staff and more experienced staff according to clinical demand. We provide a twice yearly Introduction to Intensive Care Course with extensive supernumerary time, a lecture program and competency assessments. We also have 24- hour medical cover and a full time equipment nurse.

Work/Family balance

We have self- rostering and rosters are released 6 weeks in advance. Shift lengths can be tailored to individual preference 8, 10 and 12 hour shifts are worked alone or in combination. Bank shifts are available. Full time staff are entitled to a monthly rostered day off and 6 weeks annual leave.

• 2 North

2 North is a 30 bed medical specialties ward that includes Cardiology step-down Respiratory, Gastroenterology, Endocrine, Renal, Rheumatology and HATH. 2 North caters for those patients with AMI (stable) or awaiting CAGS, PPM, AICD, heart failure, respiratory failure, tracheotomy, high flow O2, unstable diabetes, DKA, H&M as well a variety of rarely seen conditions such as Pheochromocytoma.

2 North Nursing staff has a wide variety of backgrounds, which has provided a supportive clinical environment for all staff. Staff are provided with the opportunities for professional development and skill acquisition.

Opportunities for clinical observation days, or rotations to ICU, CCU, ED and Theatre are provided as well as a comprehensive in-service program, involvement in research and projects run at ward level such as the PCI Project, (commenced 2005), supported by 2 cardiology Clinical nurse consultants facilitating the care of these patients post procedure and Division 2 scope of practice in medication administration project. Staff are encouraged to be proactive and have say in quality improvement, models of care, patient management and any other aspect that contributes to the care provision of patients on 2 North.

2 North provides flexible rostering with minimal night duty rotation through out the year.

• 3 South

3 South is a progressive and exciting 30 bed Neuro medical ward incorporating a four bed stroke Care Unit. The nursing, medical and allied health team work very closely to achieve the best possible outcomes for patients. The team is friendly, approachable and professional.

Ward 3 South aims to introduce nursing staff to an exciting area of practise. We offer supported learning through the assistance of a Neuro educator on the ward, regular inservices and learning modules.

1 East

1 East is a 30 bed orthopaedic and plastics ward. Admissions include emergency/trauma and most orthopaedic and plastics, surgical elective procedures including muscle flaps and spinal surgery. 1 East is well supported by a pain management team, wound and stomal therapy and breast care nurse roles. A healthy graduate nurse program is supported by a surgical educator and ward nurses and their Nurses Unit Managers. Staff Education programs such as Acute Care Nursing and Entry into Specialist Practice (ESP) programs are also available.

1 North -Day Surgery & 23 Hour Unit

1 North is a busy unit situated on the First Floor of Box Hill Hospital. It is the admsissions centre for the hospital, handling clerical and nursing admissions for all elective surgery patients, as well as day medical patients. Throughput in the department can be up to 40 patients per day.

1 North is staffed by skilled Registered Nurses - Division 1 who care for day case patients recovering after theatre, with a recovery period of approximately 2 hours. The 23 hour facility within the unit cares for up to 8 patients who require a longer period of recovery after theatre, and are able to be discharged the day following surgery. We have up to 4 medical patients per day who receive medical infusions over a period of up to 6 hours. These patients are accommodated in recliners in the unit. MediHotel is available for patients who are from the country, or who have no carer available at home for the night after the operation. 4 beds are available for this Monday-Thursday.

We have a supportive team approach to our nursing care, and encourage education and professional growth. Gradute Nurses form part of our team on a 4 monthly rotation.

1 South

1 South is a Surgical Ward operating 24 hours a day, 7 days a week. It specialises in Urological, Gynaecological, ENT and general surgery. It has built up these specialities over a number of years and is now moving into more complex ENT surgery. The Urological speciality is supported by Urology Clinical Nurse Consultants.

The average length of stay for patients is approximately 4 days and they are also supported by a comprehensive allied support. There are opportunities to further develop specialist clinical competencies through education and clinical experience.

1 South is well supported by a pain management team, wound and stomal therapy and breast care nurse roles. A healthy graduate nurse program is supported by a surgical educator and ward nurses and their Nurses Unit Managers. Staff Education programs such as Acute Care Nursing and Entry into Specialist Practice (ESP) programs are also available.

2 South

2 South is a 32 bed ward specialising in vascular and colo-rectal and thoracic surgery. Liver surgery is a new and exciting specialty. This ward is well supported by a pain management team, wound and stomal therapy and breast care nurse roles. A healthy graduate nurse program is supported by a surgical educator and ward nurses and their Nurses Unit Managers. Staff Education programs such as Acute Care Nursing and Entry into Specialist Practice (ESP) programs are also available.

3 North

Ward 3 North is an innovative medical ward. Our areas of specialty include infectious diseases, ACE (acute care of the elderly) and other diverse medical presentations. A team-nursing environment exists – dividing the ward into 3 sections, each focusing on a specialty area.

3 West

3 West is a 22 bed acute oncology and haematology unit that specialises in the treatment of both solid and haematological malignancies. We offer a wide range of treatments including chemotherapy, radiotherapy, autulogous stem cell support, and palliative care. We pride ourselves on the high skill level of our nursing staff who enjoy a close working relationship the unit's medical staff. A strong teamwork approach is adopted by all staff including our experienced team of allied health staff, which allows us to offer an extensive range of services. We utilise a primary nursing model of care that brings both nursing staff and patients a great deal of satisfaction.

We offer the opportunity to undertake post-graduate education with support from our experienced nurse educator, we provide strong support structures for graduate nurses, and offer an extensive in-service education program.

Paediatrics

The Paediatric Unit is a 13 bed ward, with a combination of core staff and graduate & entry into specialised practice nurses on rotation. Core staff are required to be paediatric trained, have a postgraduate paediatric qualification or at lest 4 years paediatric experience. This unit caters to a broad range of paediatric conditions e.g. paediatric medicine, surgery, infectious diseases, orthopaedics, plastics and ENT. There is an active staff education program including orientation, mentoring and preceptoring. Ongoing education is encouraged and supported.

Upton House

Upton House is a 24-bed adult psychiatric in-patient unit with a 4-bed high dependency unit servicing the Central East region. We provide a psychiatric treatment in a safe, secure environment for people with acute mental illness. Our patient care is based upon a multidisciplinary team approach to patient management.

Operating Suite

The Operating Suite consists of 6 theatres and 1 Endoscopy Suite. Box Hill Hospital offer a wide variety of surgical specialties to include, colo-rectal, general surgery, orthopaedic, plastics, ear, nose and throat, vascular, urology obstetrics and gynaecology, thoracic and endoscopy.

Opportunities are available to nurses interested in this environment to experience peri operative nursing via a graduate or an ESP program. Nurses undertaking a post graduate qualification in peri operative nursing are encouraged and supported clinically during their period of university study.

Medical Imaging

The Medical Imaging department is an integral element of the service offered by Box Hill Hospital. It is staffed by more than 80 radiographers, nurses, ward support and clerical staff. Modalities include general radiography, CT, ultrasound, fluoroscopy, nuclear medicine, MRI, vascular and cardiac angiography. The nursing staff provide an acute recovery service, scrub/scout in both the vascular and cardiac suites with diagnostic and interventional procedures being performed. Other areas of intervention that nurses participate in include, urology, obstetrics, gynaecology, respiratory, oncology, paediatrics, gerontology, rehabilitation, emergency and outpatient services. The nursing team is advanced life support trained and is dedicated to providing a quality Imaging service.

RAMU (Rapid Assessment Medical Unit)

RAMU is a 23 bed Rapid Assessment Medical Unit that caters for medical patients who require admission to hospital via our busy emergency department. Our aim is to assess, each individual holistically to ensure the best possible outcome in the shortest span of time. Our nursing staff pride themselves in attention to detail and a well honed clinical gaze, with the ability to think on the run. We practice team nursing within a wide multidisciplinary team . We are expert communicators, thrive in challenging situations, and are above all happy to come to work.

We offer fantastic opportunities to develop new skills in all areas of acute medical nursing. We are dynamic and skilled and above all can be seen still smiling at the end of the shift.
