Inpatient
Rehabilitation and Aged Care
Angliss Community Rehabilitation provides the ideal environment for recovery from an accident,
injury, or medical condition. Individual therapy is provided in the
home and in group programs on-site at both the Ferntree Gully and Croydon
sites.
Services
are provided to clients in the Community who meet Community Rehabilitation
Centre criteria, i.e. all ages with a range of diagnoses. Clients must
have rehabilitation goals either due to an acute event or exacerbation
of chronic conditions.
The treating
team consists of Physiotherapy, Occupational Therapy, Speech Pathology,
Dietetics, Social Work and Rehabilitation Medicine.
We receive
referrals from:
- Acute
or Rehabilitation Hospitals
- Rehabilitation

- GPs
- Community

- Clients/carers
Referral Form
Clients
are contacted by phone upon receipt of referral to discuss the program
and determine eligibility for our service.
There is
a small charge for individual and group therapies. These will be discussed
with the client on admission to the program. In cases of financial hardship,
special arrangements regarding fees can be negotiated.
Rehabilitation
goals are set with the client, their carers, and General Practitioner.
Their program is then undertaken in the home, with Allied Health Professionals
visiting as often as required. The client is also able to participate
in relevant groups at either Upper Ferntree Gully or Croydon.
Staff Meet
regularly to monitor client progress and goals.
Once all
goals are achieved, the Home for Rehabilitation team discharge the client,
referring them to other community agencies for ongoing care.
The Program
is closely linked with the Angliss Inpatient Rehabilitation Unit and
the Intensive Home for Rehabilitation Program.
Teams are
based at Ferntree Gully and Croydon - covering the City of Knox, Maroondah
and parts of Yarra Ranges.
