Referral Form

Podiatry is provided to those patients attending CRC/RIH as required. We do not provide community podiatry as a "lone" service.

A Medical Referral is required

 Patients are seen by appointment at:

 Overview of Service

1. To provide a multi disciplinary assessment and treatment service
2. To provide individual treatment programmes appropriate to patients needs
3. To promote a team approach and inter-departmental communication in patient care plans
4. To review all patients at regular intervals
5. To offer appropriate discharge plans
6. To consider the needs of carers and offer appropriate support
7. To provide or assist with patient, staff and community education
8. To develop awareness of community care agencies, which will enable a co-operative working relationship
9. To engage in multi-disciplinary Quality Improvement Activities
10. To evaluate the Community Rehabilitation Centre and Rehabilitation in the Home programmes in terms of their service and outcomes

Special educational information is made available from therapists depending on the patients condition and needs.
