Aged Care Assessment Service

Assisting older people and their carer's identify a type of care to suit their needs. 

Community Rehabilitation Program

Offering short term, goal oriented therapy aimed at improving clients’ function and well being following a recent stay in hospital.

Complex care clinic

Assisting with the management of elderly people with complex medical needs. 

continence clinic

Specialising in incontinence and other bladder and/or bowel function difficulties.

Falls and Balance clinic

Providing full assessment of older people experiencing falls, balance problems or gait problems.

Hospital Admission Risk Program

Helping people better manage and maintain their own health.

Post Acute Care

Coordinating short-term community support after discharge from hospital.
