Community rehabilitation offers short term, goal oriented therapy aimed at improving clients’ function and well being following a recent stay in hospital.

Our service is facilitated from Angliss Hospital, Peter James Centre, Wantirna Health, and Yarra Ranges Health.

Programs are offered at our locations or at home depending on individual therapy needs by a multi -disciplinary team.
