What is Transition Care?
Transition Care supports older people to complete rehabilitation, and address longer term care needs at home or in residential care following a hospital admission.
The program includes low intensity therapy and personal/nursing care at home or in a residential facility as part of an ongoing but slower recovery process. It provides time and assistance for clients and their family or carer to consider long-term care arrangements.
Transition care can be provided for a period of up to 12 weeks. The average period of care is expected to be approximately eight weeks.
To be eligible for Transition Care, the client must be an older person and an inpatient of a hospital, having completed their acute stay and any necessary sub-acute care (rehabilitation).
